If interested in the therapeutic shoes for people with diabetes program you will need to have a foot exam by your primary care physician [who must be a M.D. or D.O. ] before you come in. And if you have these four forms printed out and signed by your doctor it will expedite the billing process. This is only needed if we are billing insurance.
